Parents of Students Enrolling in Pre-K, K, and 6th grade, or Students Transferring into the School District:

The requirements have to be completed and submitted by October 15th of the current school year or your child will be excluded from school until submitted. NO APPOINTMENTS WILL BE ACCEPTED. Pre K is now required to submit all medical documentation prior to attendance.
If transferring from another state, your child will be required to complete a physical with an Illinois provider. There is a 30 day grace period from school entry after the October 15th deadline. If you are registering your child as homeless, please know your child will NOT be excluded as they are covered by the McKinney Vento Act, however, we ask that you still make attempts to achieve these medical requirements or reach out to your building/the district nurse if needing assistance.
We have partnered with Shawnee Health to increase the access to healthcare for our students and their families. Terrier Care (Opens August 1st for appointments), Shawnee Health (open all year long) are available to see our students and their family members for physicals and immunizations, wellness visits, as well as other services.

Students Participating in Interscholastic Sports:
Students wanting to try out for any interscholastic sport are required to have a physical examination performed yearly and submitted prior to be eligible to "tryout." The student must submit either a physical exam that is marked "yes" to participate in interscholastic sports by the provider or a completed sports physical examination performed by the provider.
** Please note if your child is entering 6th grade, only the IL Certificate of Child Health examination form will need to be completed to qualify for both the physical exam compliance and to participate in interscholastic sports as long as the doctor has completed the appropriate box.
Students With Medical Related Needs
ASTHMA**Parents of students diagnosed with asthma, the school will need an emergency asthma action plan that has been written by the student's provider or prescribing provider as well as providing the inhaler to be self-carried or placed in a central location for use at school.
SEVERE ALLERGIES**Parents of students with severe allergies requiring an Epi Pen prescription, the school will need an emergency action plan that has been written by the student's provider or prescribing provider as well as providing the epi pen to be self-carried or placed in a central location for use at school.
HEALTH RELATED NEEDS**If your student has diabetes, seizures, or any other health related needs requiring resources such as a glucometer, testing strips, lancets, etc. you are responsible for supplying all supplies. ALL EMERGENCY MEDICATION MUST BE PRESENT PRIOR TO THE CHILD ATTENDING THE SCHOOL SETTING
EMERGENCIES**We have undesignated albuterol, epinephrine, and Narcan pre standing orders in all buildings. In the event of an emergency such as an asthma attack or allergic response, the building nurse or trained staff, will administer these medications while parents are being notified and an ambulance is being called, if needed.
**Food substitutions for milk are no longer required unless an allergic reaction is the reason. District 95 now offers a milk substitute.
**IDPH Mandated Vision and Hearing Screening in Illinois Schools/Preschools
- All mandated screening must be done by screeners trained and certified by IDPH. Certificates must be current, not expired.
- All mandated screening must be done using approved instruments and test protocol as specified in the Illinois Administrative Code, Title 77, SEctions 675 & 685.
- Hearing screenings shall be provided annually for all preschool children 3 years of age (and older) in any public educational program for all children who are in kindergarten, first, second, and third grades; in all special education classes; referred by teachers; and transfer students.
- In lieu of mandated hearing screening services required in the Rules, a completed and signed report form, indicating that the child has had an ear examination by a physician and an audiological evaluation completed by an audiologist within the previous 12 months is acceptable.
- Vision screenings shall be provided annually for all preschool children 3 years of age (or older) in any public educational program for all children who are in kindergarten, second and eighth grades; in all special education classes; referred by teachers; and transfer students.
- In lieu of mandated vision screening services required in the Rules, a completed and signed report form indicating that an eye examination by an M.D specializing in diseases of the eye or a licensed optometrist has been administered within the previous 12 months, is acceptable.
