Pre-Kindergarten
A school physical exam on the IDHS form signed by a Physician, Nurse Practitioner, or Physician Assistant, with parent/guardian completing and signing the health history part of the physical on the upper portion of the form and the following immunizations are both required by October 15 of current school year or student will be excluded from school until completed and submitted to the school. Exam must have been completed within 1 year prior to the start of school.( Pre-K)
Varicella - 1 dose; on or after 1st birthday
MMR - 1 dose; on or after the 1st birthday but prior to 24 months of age.
Hep B - 3 doses; 3rd dose must have been adminstered on or after 6 months of age (168 days)
Polio - 3 doses by 2 years old; 1st two doses by 1 year of age and one additional does by 2nd birthday
DTaP - 4 doses; 3 doses by 1 year of age and one additional booster dose by 2nd birthday.
HIB - 4 doses with series; Children 24-59 months without the series must have one dose after 15 months of age.
Pneumococcal - 4 doses with series; Children 24-59 months without the series must have one dose after 24 months of age.

Kindergarten or First Entry into a School
A school physical exam on the IDHS form signed by a Physician, Nurse Practitioner, or Physician's Assistant, with parent/guardian completing and signing the health history part of the physical on the upper portion of the form and the following immunizations are both required by October 15 of current school year or student will be excluded from school until completed and submitted to the school. Exam must have been completed within 1 year prior to Kindergarten or First Grade if First Entry into school
Varicella - 2 doses
MMR - 2 doses
Hep B - no requirement
Polio - 4 or more doses with the last dose qualifying as a booster and received on or after the 4th birthday
DTaP - 4 or more doses witht he last dose qualifying as a booster and received on or after the 4th birthday
Other requirements for Kindergarten students include:
Dental Examination by May 15th (District offers exams through SIU Dental Hygiene Program in the Spring)
Vision Examination by an Eye Doctor submitted by May 15 of current school year, not included in the the IL Physical exam.
Lead Screening Assessment completed by physician included with IL physical exam, required
2nd -5th Grade
A school physical exam on file from the IDHS dated from either kindergarten exam requirement or most current signed by a Physician, Nurse Practitioner, or Physician Assistant with parent/guardian completing and signing the health history part of the physical on the upper portion of the form and the following immunizations are both required by October 15 of current school year or student will be excluded from school until completed and submitted to the school.
Varicella - 2 doses
MMR - 2 doses
Hep B - no requirement
Polio - 3 or more doses with last dose qualifying as a booster and received on or after 4th birthday, if given in series 4 or more doses required.
DTaP - 3 or more doses with last dose qualifying as a booster and received on or after 4th birthday.

6th Grade
A school physical exam on the IDHS form, signed by a Physician, Nurse Practitioner, or Physician Assistant, with the parent/guardian completing and signing the health history part of the physical form and the following immunizations are both required by October 15 of current school year or student will be excluded from school until completed and submitted to the school. Exam must have been completed within 1 year prior to 6th grade entry to be accepted.
Varicella - 2 doses
MMR - 2 doses
Hep B - 3 doses administered at recommended intervals
Polio - 3 or more doses with last dose qualifying as a booster and received on or after 4th birthday. If given in series must have 4 or more doses.
Tdap- 1 dose required for student entering grades 6th-8th.
Meningococcal- 1 dose entry for 6th or 7th grade.
DTap-3 or more doses with last qualifying as booster if received on or after the 4th birthday.

Religious and Medical Exemptions; A new exemption is required to be completed at each of the following grade levels: Kindergarten entry, 6th grade entry, or at first time enrollment or re-enrollment into our District.
Parents/Guardians who choose to not have the required vaccinations or examinations due to religious beliefs must complete and submit to the school an "Illinois Certificate of Religious Exemption to required immunizations and or examination" with detail specific religious beliefs which conflicts with the specific immunization and or exam and must by signed by a physician in order to be accepted by the district.
Parents/Guardians who choose to not have the required vaccination due to medical exemption must submit to the school a physician's statement indicating student's medical condition that contraindicates the specific immunity, be written by a MD, DO, APN,or PA that conducted the health exam, and student will be expected to receive required vaccine(s) should the medical condition change and allow immunization.
New and Transfer Students Are required to follow the grade specific schedule for compliance; will have an additional 30 days from their enrollment after the October 15 deadline of current year or will be excluded from school until completed and submitted to school.
Students from other Countries, must have an IL physical exam and shall be completed within one year prior to the date of entering school with the required immunization schedule for that grade entry.
