District-wide Registration for the 2010-2011 School Year will be held on

Tuesday, August 3, 2010,

from Noon-6 p.m.,

at the Carbondale Middle School,

1150 E. Grand Avenue.

 

Special note for Parents/Guardians of District #95 children: If your child attended District #95 last year and you registered your child at early registration in June, you do not need to attend registration in August.

 

For Parents/Guardians of children new to District #95: Please find below registration forms that may be downloaded (after July 16, 2010) and completed prior to registration. You may be asked to complete additional forms at registration.  

 

Additionally, parents/guardians of students new to the district will need to provide:

  • Original birth certificate with official seal.
  • Proof of residency (this may include an apartment lease, deed for the purchase of a home or a utility bill). If you have any questions about what to bring to registration, please contact the district office at 618-457-3591 x2101.
  • Current physical examination, immunization record and dental exam. (An explanation of these requirements may be found on this website under "DISTRICT NURSE.")
  • The address of the last school your child attended so we may, with your permission, request your child's academic records.

For your convenience, please find below registration forms that may be downloaded, completed and brought to Central Registration for all students on Tuesday, August 3, 2010. Updated forms for school year 2010-2011 will be available July 16, 2010. Please note that while we have tried to make as many of our registration forms as possible available for download, you may be required to complete additional forms at registration. Currently the Free/Reduced Lunch forms are unavailable for download.

2. 2010-2011 Registration Card (2 pages)

3. 2010-2011 Internet Use Agreement (2 pages)

4. Race/Ethnicity Form (2 pages: Letter of Explanation plus the required form)

5. Student Health Requirements (for informational purposes only)

6. 2010-2011 Home Language Survey 

7. 2010-2011 Free/Reduced Lunch Forms (If applicable. Please complete only 1 application per household, even if your children are in different attendance centers.)

If you are a parent/guardian of a returning student and you were unable to attend early registration in June, you may still complete the packet of registration materials sent home with your child in May and bring these with you to registration in August.
